Moving F.A.S.T. Class Certificate Request

Use this form to submit a completed Moving F.A.S.T. class roster for certificate review. This page is intended for facilitators, program coordinators, or approved class administrators submitting students who completed the program.

Submitting this form does not automatically issue certificates. TEA will review the class information, participant roster, and completion details before certificates are prepared or released.

Before You Submit

Enter the facilitator and class information carefully.
Enter each student name exactly as it should appear on the certificate.
Do not include Social Security numbers, birth dates, medical details, or other sensitive personal information.
Only submit students who completed the Moving F.A.S.T. class.

Review Process

The facilitator submits the class roster.
TEA reviews the submission.
Completion details are verified.
Certificates are prepared only after review.
Certificates are manually released or emailed after approval.
